I have the one with the 1st back cover image.
Pages are 62 numbered, 2 in the front not (title, TOC) and 16 map pages in center of book. I also have besides the map an order form and product summery page that has the 2nd address on it.

That is same as the one I have listed above, I didn't flip through it so I missed the front 2 and middle map section (I should check the other books again too).

Is your order form the same as the one I posted in the last image? I only posted the back side of it and not the product summary on the front side for comparison with the form posted in the Carse page.

Yep, same order form...did they come with both of your books or only the one posted without the sticker on the address?

I only received it with the one without the sticker. They were both bought 2nd hand so the sheet may have been lost if it was initially included.

In looking over my Chaosium Thieves' World boxed set, I see that Midkemia Press is given credit for some of the material inside the boxed set too. I will have to check if these tables are taken directly from one of their other books.

EDIT: The tables in the Game Master's Guide for Sanctuary definitely resemble the tables from the Cities book but some revisions and modifications have been made.
