Probably a fourth printing. This printing differs from the ones above in several ways, the bigs ones being an expanded copyright inside cover and the inclusion of a Table of Contents inside the rear cover. The dungeon map, which was inside the rear cover, has been moved to the last page. The territory map, which had been on the last page in the print above, has been left out.

mmmhhh... with ref to the last post (supposed 4th print), i have some doubts: i have a copy of the book with the features of the 3rd print (long copyright notice on 2nd page of front cover) but still the dungeon map in the rear page and no presence of ToC at all... maybe mine is a 4th print and this one is a fifth print: do you find it reasonable?

Obviously, yes. There could a dozen prints of this one. It is the nature of this enterprise to have holes. Please fill this one if you can. It's the only chance we have of getting it straight. There can never be a true print-run determination for any collectible. All we can do is make a good guess.
