A slightly older (November 1980) Retail Order Blank recently surface on the Acaeum:

This one is just a single sheet, front and back shown above. As you can see, the first page is numbered "1", and the back has the order form, so it appears to be complete, despite being much shorter than the 1981 Blanks.

The WoG Figures from the front of this sheet are all also found in the later June 1981 Blanks.
